Tambree "Tammy" L. Smiddy 41 of Leitchfield passed away on Monday June 8, 2015 at Twin Lake Regional Medical Center.
She was preceded in death her mother Sheila Smiddy.
She is survived by her children Allexxis Sinkhorn (Aaron) of Louisville, Bretta Smiddy of Leitchfield, Mikayla Wholney of Louisville and Rayna; her father Larry Smiddy (Nora) of Leitchfield; her sister Angela Smiddy of Louisville and her grand daughter Chelsea Blanford.
The family has chosen cremation with a memorial service to take place at a later date.
