Steven Ray Phelps 37 of Shepherdsville passed away Friday January 29, 2016 at Kindred Hospital in Louisville.
He was a native of Louisville and a concrete finisher with Clifford's Concrete.
He was preceded in death by his mother Minnie Sue Stepp.
He is survived by his wife Lisa Phelps, his children Brooklynn Phelps, Kalli Sue Phelps, Alexia Phelps and Ali Phelps all of Shepherdsville, his sister April Morris of Radcliff, his grandmother Helen Stepp of Brooks and a grandchild Tyler Merrifield.
Funeral Service will be 11 a.m. Wednesday at Creech Funeral Home in Middlesboro, Kentucky with burial to follow in Alva Memorial Gardens in Pathfork, Kentucky.
Visitation will be at Maraman Billings Funeral Home on Monday from 2-8 p.m. and at Creech Funeral Home on Wednesday from 9 a.m. till 11 a.m.
